Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows elevated creatinine, suggesting impaired renal function, and significant proteinuria, glycosuria, and ketonuria, indicating possible diabetic nephropathy or another renal pathology despite low HbA1c. Urine is abnormal in appearance and color, with low specific gravity, but no evidence of infection. Mild metabolic acidosis is suggested by low CO₂. Further nephrological evaluation and assessment for underlying causes (e.g., diabetes, dehydration, or other renal disease) are recommended.
