Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows persistent renal dysfunction (elevated creatinine), chronic anemia (low hemoglobin, hematocrit), and leukopenia. There is mild, fluctuating hypoglycemia and occasional mild transaminase elevation. Electrolytes and liver synthetic function are largely preserved. The findings suggest chronic kidney disease with associated anemia and possible bone marrow suppression; further clinical correlation and hematology/nephrology evaluation are recommended.
