Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows persistently elevated creatinine, marked proteinuria, glycosuria, and ketonuria, with low urine specific gravity and cloudy, brown urine. These findings indicate chronic kidney dysfunction with nephrotic-range proteinuria and possible tubular damage. The presence of glycosuria and ketonuria despite normal blood glucose suggests possible renal glycosuria or impaired glucose handling. No evidence of urinary tract infection is seen. Further nephrological evaluation is strongly recommended.
