The patient shows persistently elevated serum creatinine, marked proteinuria, glycosuria, and ketonuria, with low urine specific gravity and abnormal urine appearance, suggesting significant renal impairment (likely chronic kidney disease) with possible tubular dysfunction. Liver and electrolyte parameters are within normal limits. No evidence of urinary tract infection. Recommend nephrology evaluation and further assessment for underlying causes, including diabetes and possible nephrotic syndrome.
