The patient shows persistently elevated serum creatinine, proteinuria, glucosuria, and low urine specific gravity, with normal blood glucose and no evidence of urinary tract infection. These findings suggest impaired renal function, possibly chronic kidney disease with tubular involvement (e.g., Fanconi syndrome). Recommend nephrology evaluation and further workup for underlying etiology.
