Conclusion and Recommendations based on this report and previous findings known to us
The patient shows persistent hyperglycemia with normal HbA1c, markedly elevated creatinine, significant proteinuria, and abnormal urine findings (cloudy, brown, low specific gravity, protein, glucose, ketones, bilirubin). Lipid profile is also elevated. These findings suggest chronic kidney disease with nephrotic-range proteinuria, likely secondary to diabetic or hypertensive nephropathy, and possible underlying metabolic syndrome. Recommend nephrology evaluation and further assessment for underlying etiology and management.
