Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows mildly elevated serum glucose and creatinine, with low total CO₂ and high chloride, suggesting possible mild renal impairment and metabolic acidosis. Urinalysis reveals significant proteinuria, glucosuria, ketonuria, and abnormal urine appearance (cloudy, brown, ammoniacal), but no evidence of infection. HbA1c is normal, indicating no chronic hyperglycemia. Findings are suggestive of acute or subacute renal dysfunction with possible tubular involvement; further nephrological evaluation is recommended.
