The patient shows persistently elevated serum creatinine, proteinuria, and low urine specific gravity, indicating chronic kidney dysfunction. Mildly elevated fasting glucose with normal HbA1c suggests impaired fasting glucose but no established diabetes. Urinalysis reveals protein, glucose, ketones, and abnormal appearance, supporting renal impairment. Lipid profile is mildly abnormal. No evidence of urinary tract infection. Recommend nephrology evaluation and monitoring of renal function and metabolic parameters.
