Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows markedly elevated creatinine, proteinuria, glucosuria, ketonuria, and abnormal urine appearance, suggesting significant renal dysfunction, possibly acute or chronic kidney disease with impaired filtration. Low urine specific gravity may indicate impaired concentrating ability. Hemoglobin A1c is unexpectedly low, which may reflect analytical or clinical issues (e.g., anemia, hemoglobinopathy). No evidence of urinary tract infection. Prompt nephrology evaluation and further assessment of glycemic status are recommended.
