Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows elevated serum creatinine, mild metabolic acidosis (low CO₂), and significant proteinuria with bilirubinuria, glucosuria, and abnormal urine appearance (cloudy, brown, ammoniacal). These findings suggest impaired renal function, possibly chronic kidney disease, with evidence of tubular dysfunction and possible hepatic involvement. No signs of urinary tract infection are present. Further nephrological and hepatological evaluation is recommended.
