Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows elevated serum creatinine, significant proteinuria, glycosuria, and ketonuria, with low urine specific gravity and abnormal urine appearance, suggesting possible renal dysfunction (likely glomerular) and impaired glucose handling. HbA1c is low, indicating no chronic hyperglycemia. No evidence of urinary tract infection. Recommend further nephrological evaluation and assessment for possible acute or chronic kidney disease and consideration of causes for glycosuria and ketonuria in the absence of hyperglycemia.
