Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows significantly elevated serum creatinine, indicating impaired renal function. Urinalysis reveals marked proteinuria, glucosuria, and ketonuria, with low specific gravity and abnormal urine appearance (cloudy, brown, ammoniacal), but no hematuria or leukocyturia. These findings suggest possible acute or chronic kidney disease with impaired tubular function and possible metabolic derangements. Further nephrological evaluation is recommended.
