The patient shows significant renal impairment (elevated creatinine, proteinuria), electrolyte disturbances (high potassium, chloride), and abnormal urine findings (cloudy, brown, low specific gravity, protein, glucose, ketones, bilirubin). Blood glucose and HbA1c are normal, suggesting no acute hyperglycemia. Findings are consistent with acute or chronic kidney dysfunction, possible nephrotic syndrome, and possible urinary tract pathology. Prompt nephrology evaluation is recommended.
