Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows significant renal dysfunction (markedly elevated creatinine, proteinuria), hypoglycemia with low HbA1c, and metabolic acidosis (low CO₂). Urinalysis reveals glucosuria, ketonuria, and proteinuria with abnormal urine appearance, suggesting possible acute or chronic kidney disease with glycosuria not explained by diabetes. Immediate clinical evaluation is recommended to assess for acute kidney injury, possible underlying metabolic derangement, and to rule out urinary tract pathology.
