Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows significantly elevated creatinine in both serum and blood, indicating impaired renal function, along with high urine protein, ketones, and bilirubin, and low urine specific gravity. Urine is cloudy, brown, and ammoniacal. HbA1c is low, suggesting no chronic hyperglycemia. Findings are consistent with acute or chronic kidney dysfunction, possible nephrotic syndrome, and possible hepatic involvement. Prompt nephrology evaluation is recommended.
