Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows elevated serum creatinine, suggesting impaired renal function. Urinalysis reveals proteinuria, glucosuria, ketonuria, low specific gravity, and abnormal urine appearance (cloudy, brown, ammoniacal), indicating possible renal tubular dysfunction or acute kidney injury. Blood glucose and urea are within normal limits, but HbA1c is low, possibly due to anemia or recent changes in glycemic control. No evidence of urinary infection. Recommend further evaluation of renal function and possible underlying metabolic or systemic causes.
