Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows elevated serum creatinine, significant proteinuria, glucosuria, and ketonuria, with low urine specific gravity and abnormal urine appearance (cloudy, brown, ammoniacal). HbA1c is low, and blood glucose is at the lower end of normal. These findings suggest possible renal impairment with protein loss, and abnormal glucose/ketone handling, possibly due to acute or chronic kidney disease and/or metabolic disturbance. Further nephrological and metabolic evaluation is recommended.
