Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows elevated serum creatinine, significant proteinuria, glycosuria, and ketonuria, with low urine specific gravity and cloudy, brown urine, suggesting possible renal dysfunction (likely glomerular) and impaired tubular reabsorption. Low HbA1c with normal blood glucose argues against chronic hyperglycemia. Mildly decreased CO₂ may indicate mild metabolic acidosis. Urine findings are not typical for infection. Recommend further nephrological evaluation for possible acute or chronic kidney disease and assessment for non-diabetic causes of glycosuria and proteinuria.
