Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows markedly elevated serum creatinine, indicating significant renal impairment. Urinalysis reveals proteinuria, glucosuria, ketonuria, and bilirubinuria, with low specific gravity and abnormal urine appearance, suggesting possible nephrotic syndrome or acute kidney injury with tubular dysfunction. No evidence of urinary tract infection. Liver enzymes and electrolytes are within normal limits. Immediate nephrology evaluation is recommended.
