Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows significant renal impairment (elevated creatinine), mild hyperkalemia, hyperchloremia, and low blood glucose and HbA1c, suggesting possible hypoglycemia. Urinalysis reveals proteinuria, ketonuria, glucosuria, low specific gravity, and abnormal urine appearance (cloudy, brown, ammoniacal), indicating possible nephrotic syndrome, renal dysfunction, and/or dehydration. No evidence of urinary infection. Clinical correlation and urgent nephrology evaluation are recommended.
