Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows elevated creatinine, proteinuria, glycosuria, ketonuria, and low urine specific gravity, suggesting possible renal impairment and abnormal glucose handling despite low-normal blood glucose and low HbA1c. Urine is cloudy, brown, and ammoniacal, but no evidence of infection (negative nitrites/leukocytes). Elevated chloride may indicate mild metabolic disturbance. Recommend further nephrological and endocrinological evaluation for possible renal dysfunction and atypical diabetes or tubular disorder.
