Conclusion and Recommendations based on this report and previous findings known to us
The patient shows markedly elevated serum creatinine, indicating significant renal impairment. Urinalysis reveals proteinuria, glucosuria, and marked ketonuria, with low specific gravity and abnormal urine appearance, suggesting possible nephrotic syndrome or acute kidney injury, possibly with a component of diabetic or metabolic decompensation. No evidence of urinary tract infection. Liver and electrolyte parameters are within normal limits except for mildly elevated chloride and low alkaline phosphatase. Immediate clinical evaluation and nephrology consultation are recommended.
