The patient shows elevated serum creatinine, low total CO₂, and significant proteinuria, glycosuria, and ketonuria, with abnormal urine appearance (cloudy, brown, ammoniacal). These findings suggest impaired renal function with possible metabolic acidosis and abnormal glucose handling, despite low HbA1c. Urine findings may indicate tubular dysfunction or acute kidney injury. Further nephrological evaluation and clinical correlation are recommended.
