The patient shows markedly elevated creatinine in both serum and blood, indicating significant renal impairment. Urinalysis reveals proteinuria, glucosuria, ketonuria, low specific gravity, and abnormal urine appearance, suggesting possible nephrotic syndrome or diabetic ketoacidosis, though HbA1c is low. Lipid profile shows low total and HDL cholesterol. Immediate clinical evaluation for acute or chronic kidney disease and metabolic disturbances is recommended.
