Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows elevated serum creatinine, suggesting impaired renal function, and mild hyperchloremia. Urinalysis reveals significant proteinuria, glucosuria, low specific gravity, and abnormal urine appearance (cloudy, brown, ammoniacal), but no evidence of infection (negative nitrites/leukocytes). Findings are consistent with possible chronic kidney disease and renal glycosuria; further nephrological evaluation is recommended.
