| The patient shows markedly elevated creatinine, proteinuria, low urine specific gravity, and abnormal urine appearance, suggesting significant renal impairment, possibly chronic kidney disease. Metabolic acidosis is indicated by low CO₂ and high chloride. There is also glucosuria and ketonuria despite low-normal blood glucose and low HbA1c, which may suggest renal glycosuria or tubular dysfunction. Further nephrological evaluation is strongly recommended. |
