Conclusion and Recommendations based on this report and previous findings known to us
The patient shows markedly elevated creatinine, proteinuria, low urine specific gravity, and abnormal urine appearance, suggesting significant renal impairment, possibly chronic kidney disease. Metabolic acidosis is indicated by low CO₂ and high chloride. There is also glucosuria and ketonuria despite low-normal blood glucose and low HbA1c, which may suggest renal glycosuria or tubular dysfunction. Further nephrological evaluation is strongly recommended.
