Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows mildly elevated creatinine, low total CO₂, and significant proteinuria, ketonuria, and glucosuria, with low urine specific gravity and abnormal urine appearance (cloudy, brown, ammoniacal). HbA1c is low, suggesting no chronic hyperglycemia. Findings indicate possible acute or chronic kidney dysfunction with impaired concentrating ability and possible tubular damage. Urine findings may also suggest urinary tract pathology. Clinical correlation and further nephrological/urological evaluation are recommended.
