Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows hypoglycemia with low HbA1c, elevated creatinine suggesting impaired renal function, and significant proteinuria with low urine specific gravity, indicating possible nephrotic syndrome or glomerular pathology. Urine findings (cloudy, brown, ammoniacal) may suggest urinary tract involvement, but no evidence of infection (negative nitrites/leukocytes). Recommend clinical correlation for renal and metabolic status and further nephrological evaluation.
