Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows elevated creatinine, proteinuria, glycosuria, ketonuria, and abnormal urine appearance, suggesting significant renal dysfunction, possibly with tubular involvement. Low CO₂ may indicate metabolic acidosis. Low HbA1c with glycosuria and ketonuria could reflect recent-onset or poorly controlled diabetes or another metabolic disturbance. Dyslipidemia and mild anemia are also present. Prompt nephrology and endocrinology evaluation are recommended.
