Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows markedly elevated creatinine, proteinuria, glucosuria, and ketonuria, suggesting significant renal impairment with possible nephrotic-range proteinuria and metabolic derangements. Low total CO₂ may indicate metabolic acidosis. Urine findings (cloudy, brown, low specific gravity) further support renal dysfunction. HbA1c is low, making chronic hyperglycemia less likely. Recommend urgent nephrology evaluation and further assessment for acute or chronic kidney disease and possible underlying causes.
