Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows elevated creatinine, proteinuria, glucosuria, and ketonuria, suggesting impaired renal function and possible diabetic or metabolic disturbance, despite low HbA1c. Urine is cloudy, brown, and has low specific gravity, indicating possible renal concentrating defect or tubular dysfunction. No evidence of infection (negative nitrites/leukocytes). Recommend further evaluation for renal impairment, possible acute or chronic kidney disease, and assessment for underlying metabolic or endocrine disorders.
