Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows elevated serum creatinine, mild hyperchloremia, and significant proteinuria with low urine specific gravity, glucosuria, and bilirubinuria. Urine is cloudy, brown, and ammoniacal, but without nitrites, leukocytes, or hematuria. These findings suggest possible renal impairment (likely chronic), with tubular dysfunction (proteinuria, glucosuria) and no clear evidence of urinary tract infection. Further nephrological evaluation is recommended.
