Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows markedly elevated creatinine, significant proteinuria, glycosuria, and ketonuria, with low urine specific gravity, suggesting severe renal impairment, likely chronic kidney disease with possible nephrotic features. The low HbA1c and normal blood glucose argue against diabetes as a primary cause. Urine findings (cloudy, brown, ammoniacal) may indicate underlying urinary tract pathology. Prompt nephrology evaluation is recommended.
