Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows significantly elevated serum creatinine, indicating impaired renal function. Urinalysis reveals marked proteinuria, glucosuria, and ketonuria, with low specific gravity and abnormal urine appearance (cloudy, brown, ammoniacal), but no evidence of infection (negative nitrites/leukocytes). These findings suggest acute or chronic kidney dysfunction, possibly with nephrotic features and poor urine concentrating ability. Further nephrological evaluation is recommended.
