Conclusion and Recommendations based on this report and previous findings known to us
The patient shows elevated serum creatinine, proteinuria, glucosuria, low urine specific gravity, and abnormal urine appearance (cloudy, brown, ammoniacal). These findings suggest impaired renal function, possibly chronic kidney disease with tubular dysfunction. No evidence of urinary tract infection (negative nitrites/leukocytes). Further nephrological evaluation and assessment for diabetes or other causes of renal impairment are recommended.
