Conclusion and Recommendations based on this report and previous findings known to us
The patient shows marked renal dysfunction (elevated creatinine in blood and urine, significant proteinuria, and high urine microalbumin/creatinine ratio), suggestive of nephrotic syndrome or advanced kidney disease. Hypoglycemia and low HbA1c are noted. Urinalysis reveals glucosuria, ketonuria, and abnormal urine appearance, raising concern for possible diabetic ketoacidosis or other metabolic derangement. Immediate clinical evaluation and nephrology referral are recommended.
