Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows elevated serum creatinine, suggesting possible impaired renal function. Urinalysis reveals significant proteinuria and glucosuria, with low specific gravity, but no hematuria or leukocyturia. Urine is cloudy, brown, and ammoniacal. These findings may indicate renal tubular dysfunction or early diabetic nephropathy; further nephrological evaluation and assessment for diabetes are recommended.
