Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows elevated serum creatinine, suggesting possible renal impairment. Urinalysis reveals proteinuria, glucosuria, low specific gravity, and abnormal urine appearance (cloudy, brown, ammoniacal), supporting renal dysfunction. Blood counts are largely normal except for elevated hemoglobin and platelet distribution width. No evidence of urinary infection. Findings are consistent with possible chronic kidney disease; further nephrological evaluation is recommended.
