Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows markedly elevated creatinine and microalbumin/creatinine ratio, indicating significant renal impairment. Urinalysis reveals proteinuria, bilirubinuria, glucosuria, ketonuria, low specific gravity, and abnormal urine color/appearance, supporting renal dysfunction. Mild anemia is present. HbA1c is low, suggesting no chronic hyperglycemia. No evidence of infection. Recommend urgent nephrology evaluation for possible acute or chronic kidney disease and further assessment of underlying etiology.
