Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows markedly elevated serum creatinine and proteinuria, indicating significant renal impairment. Mildly elevated ALT suggests possible hepatic involvement. Urine findings (glucosuria, ketonuria, proteinuria, brown color, low specific gravity) point to impaired renal concentrating ability and possible metabolic derangement (e.g., diabetes or ketoacidosis). Further nephrological and metabolic evaluation is strongly recommended.
