Conclusion and Recommendations based on this report and previous findings known to us
---
The patient shows markedly elevated creatinine in both serum and blood, indicating significant renal impairment. Urinalysis reveals heavy proteinuria, glucosuria, ketonuria, low specific gravity, and abnormal urine appearance, suggesting nephrotic-range proteinuria and possible tubular dysfunction. Microalbumin/creatinine ratio is highly elevated. HbA1c is low, excluding chronic hyperglycemia. Findings are consistent with acute or chronic kidney disease with nephrotic syndrome features; urgent nephrology evaluation is recommended.
