Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows significant renal impairment (markedly elevated creatinine and urea), nephrotic-range proteinuria, and abnormal urine findings (cloudy, brown, low specific gravity, ketonuria, glucosuria, bilirubinuria). HbA1c is low, suggesting no chronic hyperglycemia. Lipid profile shows low HDL and borderline high LDL. Findings are consistent with acute or chronic kidney disease, possibly nephrotic syndrome. Immediate nephrology evaluation is recommended.
