The patient shows markedly elevated creatinine, proteinuria, glucosuria, ketonuria, and low urine specific gravity, suggesting significant renal dysfunction, likely acute or chronic kidney disease with possible nephrotic features. Low total CO₂ may indicate metabolic acidosis. Urine findings (cloudy, brown, ammoniacal) suggest possible urinary tract pathology. Low HbA1c with glucosuria and ketonuria may indicate recent-onset diabetes or another metabolic disturbance. Immediate nephrology and further metabolic evaluation are recommended.
