Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows elevated creatinine, proteinuria, ketonuria, bilirubinuria, and low urine specific gravity, suggesting impaired renal function and possible nephrotic syndrome or acute kidney injury. Urine is cloudy, brown, and ammoniacal, but no evidence of infection (negative nitrites/leukocytes). HbA1c is low, excluding chronic hyperglycemia. Recommend further nephrological evaluation and assessment for underlying causes of renal impairment and proteinuria.
