Annotation

Conclusion and Recommendations based on this report and previous findings known to us
The patient shows significant renal impairment (elevated creatinine, proteinuria, low urine specific gravity), with marked protein and ketone presence in urine. Urine is cloudy, brown, and ammoniacal, but no evidence of infection (negative nitrites/leukocytes). Mildly low globulin and AST are noted. Findings suggest chronic kidney disease with possible nephrotic-range proteinuria and metabolic derangement; further nephrology evaluation is recommended.
