Dear colleague, I am writing to inform you regarding your patient, an 81-year-old
male admitted under our care for severe aortic valve stenosis. Upon admission, echocardiographic
assessment confirmed critical aortic stenosis with significantly reduced valve area
and elevated mean transvalvular gradient. Given his symptomatic presentation and the
severity of findings, the patient was evaluated by our multidisciplinary heart team.
Following careful risk stratification and cardiac catheterization, he underwent successful
surgical aortic valve replacement without perioperative complications. Post-operative
recovery was uneventful, with satisfactory haemodynamic parameters on discharge. Current
medication adjustments have been made accordingly, and I kindly request continued
follow-up including wound inspection, anticoagulation monitoring, and cardiac rehabilitation
referral. Please do not hesitate to contact us should any concerns arise. With collegial
regards.
