Hospital course note
Dear colleague, I am writing to inform you regarding our shared patient, a 48-year-old
male admitted for pre-renal transplant evaluation. During his stay from 12th to 18th
March 2024, we completed a comprehensive transplant workup including cardiac assessment,
vascular imaging, serological screening, and renal function studies. Tissue typing
and crossmatch testing were performed, and infectious disease screening was completed
satisfactorily. Urological evaluation confirmed suitability for transplantation. After
multidisciplinary review, the patient was deemed an appropriate transplant candidate
and has been formally listed on the transplant waiting list. Dialysis preparatory
care planning has been initiated as per protocol pending organ availability. Please
continue monitoring his current conservative management and ensure timely follow-up.
