Dear colleague, I am writing to inform you regarding your patient, a 38-year-old female
who was admitted on 30.04.2012 for elective voluntary surgical sterilization. Prior
to the procedure, a thorough preoperative assessment was completed, including routine
blood work, ECG, and anaesthesiological evaluation, all yielding no contraindications.
The patient confirmed her informed and voluntary decision for permanent contraception.
On 30.04.2012, laparoscopic bilateral tubal ligation was performed under general anaesthesia
without intraoperative complications. The postoperative course was uneventful, and
the patient was discharged home on 01.05.2012 in good general condition. Wound healing
was satisfactory. The patient was counselled regarding the permanent nature of the
procedure and postoperative care instructions were provided. No further gynaecological
follow-up is required from our side. Please continue routine primary care. We thank
