Dear colleague, I am writing to inform you regarding our mutual patient, an 85-year-old
female, who was admitted on 09/03/1972 at her own explicit request for voluntary surgical
sterilization. Prior to the procedure, a thorough preoperative assessment was conducted,
including clinical examination, routine laboratory workup, and anaesthetic evaluation,
all of which confirmed her fitness for surgery. The patient was counselled extensively
regarding the permanent and irreversible nature of the procedure, and informed consent
was duly obtained. Bilateral tubal ligation was performed without intraoperative or
postoperative complications. Recovery was uneventful, and the patient was discharged
home on 10/03/1972 in good general condition. No further gynaecological follow-up
is specifically indicated, though routine primary care monitoring remains appropriate
