Hospital course note

Dear colleague, I am writing to inform you regarding your patient, a 32-year-old male admitted on 05.11.2022 due to substance dependence disorder involving multiple substances. Upon admission, a comprehensive psychiatric and somatic assessment was conducted, including structured clinical interview, toxicological screening, and laboratory workup. The patient presented with signs of active dependence syndrome and mild withdrawal symptoms. During the inpatient stay, medically supervised detoxification was initiated alongside pharmacological withdrawal management. Psychoeducational counselling sessions were provided, and the patient was stabilised over the course of the admission. At discharge on 07.11.2022, the patient was clinically stable, motivated for further treatment, and referred to an outpatient addiction medicine programme. Continued monitoring of liver function and regular follow-up appointments are strongly recommended. Please do not hesitate to contact me should further information be required.
